These are produced for doctor authorisation as soon as possible upon receipt but always allow 48 hours. Never leave your request until it is urgently needed.
Patients may only order medication listed on their repeat medication slip, which will have been agreed by the GP already. All other requests for medication must be written on the special request form available at the surgery. These require the doctors' authorisation and will take a little longer to produce. On these occasions you may also have to have a consultation to discuss your request with a doctor, please ensure you allow time for this.

Requesting your medication has never been easier
To streamline medication requests both for patients and the surgery and to ensure patients safety with issuing of the correct medication we are encouraging patients to request their repeat items online.

The first and most important reason is safety. Statistics show that 4 out of every 100 telephone calls can contain an error in the information received during a telephone request.
You can request your repeat medication by the following methods:
Order Online or use the NHS App (please speak to our Reception Team if you are not signed up) alternatively
Use your repeat slip (right hand side of green prescription) – ticking the items you need to order and handing this slip into the practice.
Sign up to a repeat ordering service with your local pharmacy/chemist
In order to maintain our high standard in prescribing, we review all our patients repeat medication regularly. Occasionally you may be asked to consult with a doctor before more medication is issued to ensure you are getting the best possible treatment. This can be done over the telephone in most cases, so you do not need to come in, but if requested, please do call to make a telephone consultation with your doctor, or we may not be able to continue to issue your medication.
- Requests can only be initiated by the patient or the carer.
- The patient has a list of the current items available on repeat.
- The patient/carer checks their current stocks of medicines before ordering any more.
- Where possible all requests should be made using the computer generated slip.
- If there is a reason for ordering early, (e.g. holiday) please indicate this on the repeat computer slip using the most recent request slip only.
- All items must be clearly marked with a tick.
- If there have been any changes to medication or doses since the repeat slip was issued please indicate the change clearly and give reason for change. Hospital discharge letters to the GP can be delayed and not arrive before the patient needs more medication.
- If it is not possible to use a repeat slip, please use the slips available from Reception and include:
- Name and address of patient.
- Name and strength of the medication.
- The dose.
- When it was last supplied.
- The repeat slips must be placed in the box at the surgery or posted - no telephone requests will be accepted.
- If a stamped addressed envelope is enclosed, the prescription will be posted back, but please allow extra time for this.
- Please allow 2 working days before collecting the prescription.
- All prescriptions for controlled drugs must be signed for on collection by the patient or authorised carer.
- If a prescription is being collected by a non-family member, please give them written permission to do so.
